What is a DD Form 877?
A DD Form 877 is a form used to request medical records. It can be delivered by a military member to an MTF. The MTF will file the document in the SF 600, a Chronological Record of Medical Care. If the SF 600 is not in electronic format, the facility will scan loose paper forms. Once the documents are scanned, they will be uploaded into the Theater Medical Data Store.
SF 600s from single-visit encounters must be filed in chronological order. This is done by completing the patient’s demographic data on each page. The date of the current food handler examination should be recorded as well. Upon completion, the records should be closed. In addition, if the patient is legally aged, they must contact the ROI office.
DD Form 877s are not for hand carry. They can be sealed in an envelope. An explanatory entry should be made on the SF 600 if the person is a deserter. Alternatively, they can be sent to an STR RPC for digitization. During this process, they will be broken into three sub-parts: Document Title, Document Type, and Document Title – STR Medical Record Part 1. Each sub-part will be indexed as an appropriate STR part. For example, if a military officer is transferring to an army unit, the Document Title will be Army Officer – STR Medical Record Part 1 and if the officer is a civilian employee, the SSN of the employee will be used.
