What is a DD Form 3039?
If you are an army member, you may be wondering what a DD Form 3039 is. While this form is used by the Army, it is also found in many other DoD forms. You may have even seen it listed as DA Form 3039. However, DA is an incorrect name, and the Army calls the form DD Form 3039. Regardless of the name, the form is available in PDF format and is used by the United States Army. It was released on January 1, 2017, and is a new version of an older form that was first published in 2006.
DD Form 3039 can be found in several other forms, but you can download this one in PDF form. If you want a hard copy, you can contact your military services. They can provide you with a paper version of the form if you don’t have access to an electronic version. There are hundreds of other forms that are similar to DD Form 3039.
