What is a DD Form 3010?

The DD Form 3010 is a military form utilized by the United States Army. It is used by cadets, enlisted personnel, and medical staff to document biological products, such as blood, urine, and other fluids. In the event of a problem, the form must be filled out in detail.

Upon completing the form, a cadet or cadre member must sign it. Afterward, the preferred contact information and other fields must be completed. A reenlistment code must also be verified.

This form should be completed within three business days. During this time, any changes must be submitted and the form reviewed by NMDP/CIBMTR. After the review, the NMDP will notify the sponsor and cord blood bank.

The date of infusion must be entered. This date should be in ISO format. If any fields are left unfilled, the form cannot be processed. Additionally, the date of receipt should be entered.

Product shipment must include product labeling and accompanying records. Moreover, the product type received by the transplant center must be noted.

For all infused products, the date of infusion must be entered. Nevertheless, the date of infusion for overnight infusions should be indicated.

Any serious adverse event must be documented on Form 3001 – Adverse Event Form.
