A Transportation Discrepancy Report is required by Government receiving activities when discrepancies are found on a carrier’s delivery receipt. The form is approved by the Office of Management and Budget. This document is used to report all discrepancies and other non-standard shipments, such as unauthorized or hazardous materials.

Standard Form 361 is a document that serves as the government’s primary method of discrepancy reporting. It must be prepared by the receiving activity and then distributed to the appropriate agency. Some supporting documents may include carrier inspection reports. If there are overages or shortages, the receiving activity must reconcile them within 15 days.

There are two new Discrepancy Codes, which will be assigned to shipments that are not associated with a National Shipping Number. These codes are outlined in Enclosing 2. For example, if a shipment is missing a stock number, the stock number will be categorized as a “stock number missing” Discrepancy Code U11. In addition, a new one-position type code will be created to identify receipts of non-DSS wholesale material, such as petroleum.

New categories of Discrepancy Codes have also been added, including Hazardous Material and Condition of Materiel. Additionally, a new disposition code is now available for SDRs that cannot be routed electronically.

SF 361 must be completed and distributed to the appropriate agency as soon as possible. For miscellaneous problems, the document should be accompanied by photos and other details.
