A Transportation Discrepancy Report is required by Government receiving activities when discrepancies are found on a carrier’s delivery receipt. The form is approved by the Office of Management and Budget. This document is used to report all discrepancies and other non-standard shipments, such as unauthorized or hazardous materials.

Standard Form 361 is a document that serves as the government’s primary method of discrepancy reporting. It must be prepared by the receiving activity and then distributed to the appropriate agency.
