What is a DD Form 491-1?
If you work for a military agency, you may have been asked to complete a DD Form 491-1. It is often confused with the DA Form 491-1, but they are different. The DD Form is used by the U.S. Army, and you can download it in PDF format.
In addition to this form, the Department of Defense issues hundreds of other DoD forms. Those forms are designed to make reporting easy and accurate. Several of them are required by Section 16. Some of them, like Form 4 and 5, may continue to be required. As such, it is important to keep up-to-date with the latest versions of those forms. You can find a list of those forms on the DoD’s website, or you can ask your local military office for a copy. Also, you can check with the DoD’s Forms Manager to see if they are currently updated.
Aside from these forms, you can also request a review of your case by the Judge Advocate General. They can review your case and make recommendations to the military commander. If you are not satisfied with their recommendation, you can file an appeal with the DoD.
