What is a DD Form 3028?
DD Form 3028 is not a new form in the Department of Defense. It is an abbreviated form of a much longer DD Form 2875. The acronym for this form is a shortened version of the DD form requesting access to a classified system. This shortened DD form can be used to adjudicate a request for access to a classified system or to request system authorization.
This shortened DD Form 3028 is only one of several similar forms available. Other examples include a DD Form 2875 requesting system authorization and a DD Form 2907 for requesting a system change. However, only DD Form 2875 and DD Form 2907 are adopted for use throughout the DoD. DISA, the office that administers the DD Form 2875, coordinates with federal entities. In addition, the Office of Management and Budget and the Washington Headquarters Services have also collaborated on this DD Form 2875.

The Department of Defense will be awarding a commemorative lapel pin to veterans who served in the Vietnam War. They will receive a commemorative certificate from the state where the veteran served and an accompanying lapel pin. Qualifying Vietnam War veterans will be invited to a ceremony where they will receive their commemorative pins. This program is part of a series of commemorative programs coordinated by the Secretary of Defense.
For those who are seeking a certificate of honor, the process is relatively simple. All you need to do is access the Commemorative Partners Portal and request a certificate. You can also order a color or black-and-white copy of your certificate. Once the information has been verified by the DoD, you will receive a printed certificate. In addition, you can also purchase a lapel pin from the Commemorative Partners portal.
If you are a surviving spouse or the immediate family member of an American service member killed in the Vietnam War, you may qualify for an Unaccounted for/Missing in Action Certificate. Alternatively, you can obtain an “In Memory of” Certificate for an American military member killed in the Vietnam War. Each certificate is available from the Commemorative Partners Portal.
