DD Form 3067-12 – SMART Scholarship Scholarship-for-Service Program Service Agreement
The DD Form 3067-12 – SMART Scholarship Scholarship-for-Service Program Service Agreement is an official document issued by the Department of Defense that outlines the terms and conditions of the SMART scholarship program. This document includes the scholarship eligibility requirements, employment obligations, and other details.
The SMART Scholarship Program awards full-tuition scholarships to undergraduate, graduate, and doctoral students pursuing studies in one of 21 academic disciplines critical to national security at regionally accredited colleges and universities in the United States. After graduating, SMART scholars must fulfill a service commitment at a DoD facility. This service commitment varies from one year to five years, depending on the length of the scholarship award.
As a part of their SMART Scholarship, scholars also receive full tuition and annual stipends while enrolled in their college or university education. In addition, they are provided internship opportunities and a guaranteed civilian job with the DoD upon graduation. In return for their monetary contributions, SMART Scholars are expected to make significant contributions to the DoD through service commitment. As a result, scholars must ensure that their study abroad activities do not interfere with the completion of the DoD internship and their post-graduate employment requirements. They are also responsible for providing the SMART program office with written confirmation from their university that the study-abroad program satisfies these requirements.
